I have been using the cream for about 7 weeks now and I love it. I have noticed a reduction in my deep cystic acne scars caused by to 30 years of acne breakouts. The Structure of Skin
To understand how BIOSKINCARE helps you get rid of adult acne (and teen acne pimples) and repairs acne lesions with scarless healing lets take a look at the structure of skin.
Sebaceous glands are attached to hair follicles and are found mostly on the face, neck, back and chest. It is this gland that produces sebum, an oily material that surrounds each hair follicle.
The fact is when sebum flows through the hair follicles to the surface of the skin it helps to lubricate our skin and protects our skin as it contains lactic acid and fatty acids, both of which inhibit the growth of some pathogenic bacteria and fungi, which are always ready to intrude especially whenever our immune system becomes impaired.
Fact is also that when sebum does not flow properly it disrupts or destroys some of the lining cells of the sebum ducts. And guess what? Our body readily reacts to the damage. Of course! It’s far from being deaf to those signals, unlike us that usually have a poor ability to listen to signs of danger or even to those warning us out loud of imminent perils.
What is trapping sebum within these sebaceous glands?
It can simply be some of the dead squamous cells from the outer stratum corneum that have fallen down into the areas around the hair follicles plugging the sebaceous glands. It may be the shear forces of a sudden outfow of sebum that injures the cells lining the sebum canals. Or it may be excess protein toxins you have been eating and have remained within your skin making your skin acne prone as they attract too much moisture which ultimately e 'pinch-off sebum glands' — OUCH! —. This causes a buildup of oil underneath the skin that gets trapped, thereby enlarging the sebaceous gland.
The sebum is a secretion which is formed by enzymatic dissolution of the whole sebaceous gland cells. Contrary to other glands, in which the secretion is formed by extrusion of a part of the glandular cell which continues to be secreted as long as the cell remains alive and functioning, the sebaceous gland is a holocrine gland in which death and dissolution of the secretory cell is required to form the secretion.
Since the secretion of sebum is a continuous process, the number of cells which undergo dissolution (lysis) to form the secretion is very high. Therefore, the corresponding number of cells have to multiply and produce cells which mature in order to form sebum. The constant flow of sebum particularly abundant when oiliness is excessive as in acne, requires the sebaceous gland cells which occupy a comparatively small space, to be highly active and to multiply at a high rate in order to provide sufficient sebum to cover comparatively much larger areas of the body surface. The cells of the sebaceous glands therefore are probably the most rapidly multiplying normal cells in the human body.
The pathological mechanism underlying acne includes a number of steps. The drying out of sebum at the external orifice of the sebaceous gland duct and/or the hair follicle leads to formation of a "plug" (comedone) which obstructs the flow of sebum. On exposure to oxygen, the comedone turns dark forming what is commonly referred to as a "black head".
The water content of the comedone is reduced by evaporation and diffusion into the adjacent horny layer (keratin) of the surface epidermis resulting in a hardening of the comedone, starting at the external surface. The comedone may become attached to the keratin and thus "moored" to adjacent elements of the skin. The comedone becomes modified chemically, as well as physically, thus becoming a material which is foreign to the body.
This state of "foreignness" should provoke an inflammatory reaction, including immune reactions and other responses of various defense systems, particularly those associated with granulocytes and macrophages. If the inflammatory reaction and the immune and other defense responses are effective in eliminating or containing the effects of the comedone, further progression of acne manifestations do not occur. Frequently, however, the immune and other defense reactions are not effective in terminating the acne process at this stage and the process progresses partly or wholly as described below.
While the comedone is obstructing the outlet of the duct, the sebaceous glands can continue to form sebum, which accumulates in the duct and in the glands, distending both. The distension and the resulting pressure lead to further intensification of the inflammatory reaction in the adjacent skin and subcutaneous tissues and produce additional swelling (edema), redness (erythema), discomfort, and a mass, which includes the obstructed and thefore encysted sebaceous gland ("redhead", "pimple", or acne papule). Frequently, the defense mechanisms are not adequate to terminate this process promptly at the acne papule stage and it continues to progress as outlined further below.
Acne Bacteria
The above conditions favor the growth of bacteria, and the resultant infections involve the duct, the sebaceous glands and the surrounding tissues, usually in that order. The onset of the infection produces further inflammatory changes, thereby initiating a vicious cycle causing continued and/or increased obstruction of the outflow of the sebum, which in turn leads to more pressure, more inflammation and continued or progressive infection. This leads to the formation of the acne pustule. The immune and other defense mechanisms having been inadequate to prevent these conditions from arising, frequently fail to arrest or reverse the process early in the acne pustule stage and it persists or progresses further.
The plugged follicles with increased sebum are a rich environment for the uncontrolled development of propionibacterium acnes (P. acnes), which is a normal part of the skin flora, and is one of the causes of inflammation and redness around acne lesions.
If the inflammation is right near the surface, you get a pustule; if it’s deeper, a papule (pimple); deeper still and it’s a cyst. If the oil breaks though to the surface, the result is a “whitehead”. Actually very little sebum infection by acne bacteria can cause the reactions described above.
If the oil becomes oxidized (that is, acted on by oxygen in the air like to an apple), the oil changes from white to black, and the result is a “blackhead.”
Anything that causes excess water retention within your skin can pinch off (ouch!) the sebaceous glands within your skin and make you break out sebum where acne bacteria will proliferate, in minutes, specially if you are going through a period of high sebum production. Excess salt and spices are the most common triggers, so if you eat too many potato chips... they contain both.
Pinpointing the Inflammatory Acne Triggers
Our body reacts in very much the same way it learned to react during the our evolutionary drift. By sending to the site of injury and insult all its army of immune system soldiers (lymphocytes, macrophages, neutrophils...) overcharged with chemical death weapons to get rid of the offenders, with no consideration even for its own cells that might happen to be in their way and also do get killed. After all... it used to be a matter of life or death!
This is what is meant by “inflammatory reaction”. And, Oh Boy! You might as well bet those soldiers are inflamed!
Now of course there are factors that "cause" hyperactivity of our glands, clogged pores, bacterial infection and the like. Stress is the hands-down number one cause of sebaceous glands hyperactivity that may turn out into acne inflammation. An there are other contributing factors as well, including picking and squeezing pimples, pressure or friction (head bands, caps etc), lifestyle, birth control pills, cosmetics, climate and diet, but stress is the Big Kahuna!
Also in areas of actual or potential hair growth, abnormalities of the hair follicles and hair may be associated with the disturbances in the sebaceous glands which are located in the immediate vicinity of the hair shaft. These glands open through their ducts into the hair root canal, thus forming a common anatomical structure, the pilosebaceous apparatus (PSA).
The sex hormones and possibly other hormones may play a role in regulating the sebaceous glands and/or the PSA. Excessive lipids placed on the skin from exogenous sources are an example of a cause which can lead to a condition resembling acne.
Now, you can not blame your body for producing large amounts of sebum. The problem is how it reacts to injury inflicted to the cells that line the sebum canals, when the canals become overcharged with sebum pressing to outflow to the surface —where it belongs—, or when they are not able to reach the skin surface because of clogged pores and pinched-off sebum glands.
Okay! You will address very oily skin and clogged pores with an acne cleansing lotion, for a while, and until your skin is back in balance. And you might as well consider changing some of your eating habits to avoid sebum canals pinched-off by the pressure of excess moisture attracted by toxins discharged within your dermis by your lymphatic system, as you may learn by visiting the Acne FAQ where we explain how to detoxify the skin from within.
Now, what about the inflammatory reaction which occurs each time those events re-occur as it is guaranteed because of your natural hormonal imbalances? Well unless you live in a perfect world, totally free of stress. Maybe not a very attractive realm though.
Counteracting the Acne Triggers
Got the point? Modulating the inflammatory response is the recipe for success! Cool down guys (immune cells), past are the times when bacteria and other micro-organism ruled. Or is it just wishful thinking? Is it not true that we carry on and inside our body three times more bacteria than the number of cells our organism is made of?
Fact is our body is ingrained with the memories of past battles, fought to protect our system whenever damaged or injured and does not listen to just any argument made up by the pharmaceutical or cosmetic companies that sell anti acne products aiming to control bacteria.
During our evolutionary drift, injuries and wounds represented a serious threat to our organism, not only due to blood loss, but also due to tissue damage or infection from the invasion of foreign bodies such as dirt, splinters and bacteria. The adult wound healing mechanism that evolved to respond to this threat has two fundamental characteristics: First, there is a rapid and robust inflammatory response, with recruitment of activated macrophages, neutrophils and lymphocytes to the injured site; and second, there is a fibrotic "walling-off" response to isolate the foreign body, with liquefaction of adjacent tissue leading to abscess formation and scarring.
In fact acne lesions are in this respect similar to any skin injury. Our system responds with the same strong inflammatory knee-jerk to the sebum that is supposed to outflow to the surface of the skin but remains trapped within the hair follicles as if it where a foreign body; or when sebum production is exacerbated and the first flow of sebum through the previously empty duct creates shear forces of sufficient magnitude that injure the walls of the sebum canals or the pilosebaseous glands.
Inflammation exacerbates when acne bacteria (Propionibacterium acnes) and other deliriously happy bacteria always present on the surface of the skin (Staphylococcus aureus, Staph. epidermidis, diphtheroids, streptococci, Candida, etc) start to proliferate uncontrolled in skin punctured or broken. The problem is magnified if right there the deliciously rich feeding fatty sebum is plenty.
Is the inflammatory response still appropriate ?
Scar Less Healing
"An abscess and its subsequent scar is not an evolutionarily optimized end point for today's injuries, Dr. Mark Ferguson, D.D.S., Ph.D., professor in the faculty of life sciences at the University of Manchester, England, says. The abscess and scarring response, with its massive inflammatory overdrive, is optimized for a very different type of skin lesion than those occurring nowadays where hygiene and clean faces and bodies rule. The abscess and scar is induced by this inappropriate inflammatory response... "
Lesions that affect the cells lining the sebum canals, arising from a sudden overflow of sebum or pinched-off sebum glands, occur in an otherwise clean skin without dirt or contamination by foreign bodies. These lesions should therefore be ideal candidates for healing, without complications, by a regenerative wound healing mechanism rather than a scarring mechanism. And the outcome should be characterized by scar less healing.
The healing process inside the hair follicles should be very much like what our system accomplishes for fetal wounds which heal quickly by regenerating cells and leaving absolutely no trace behind.
Something is missing in all those products out there.
Something that would signal your body it is being taken care of properly and does not need to overly react with its inflammatory army. Not just the natural or chemical ingredients that fight the symptoms, when it is already too late: acne bacteria gone wild, occlusion of the ducts (microcomedones) and then enlarged comedones that become inflammatory lesions.
This is a vicious cycle causing continued and/or increased obstruction of the outflow of the sebum, which in turn leads to more pressure, more inflammation and continued or progressive infection.
Such continued obstruction, with or without bacterial infection, leads to the formation of cysts. Infection of a cyst results in the formation of an abscess which leads to local tissue destruction. If this destruction of tissue has involved the connective tissue elements of the skin or subcutaneous tissues to a sufficient degree, healing is frequently accompanied and/or followed by scar formation.
Acne scars can vary from minimal to extensive and severely disfiguring problems which are permanent sequallae of acne. While the process by which acne arises and lasts for an indefinite time may and frequently does come to a halt as a result of treatment or spontaneously but the scars persist for life unless they are removed.
A Surprising Solution from Life's Genius
Fortunately we have found, by chance and keen observation, a better model. One geared as a different evolutionary immune response and cell regeneration system by a little invertebrate which secretes a substance onto its skin to keep microbes at bay and to repair it when damaged. The first surprising thing about this little creature is it has a skin made of the same structures and cellular makeuo of human skin. The same collagen, elastin and water holding molecules..

Invertebrates also respond to injury and insult from the environment by regenerating damaged cells, and even full organs and tissues, without an immoderate inflammatory reaction and yield quick results and an orchestrated skin repair and remodeling without aberrant scar healing! Yes, this is accomplished by little creatures that we, arrogant humans, many times consider an inferior form of life, in a less evolved stage.

Antimicrobial Peptides Control Blemish Breakouts
Antimicrobial peptides do not create bacterial resistance. These peptides are thought to punch holes in the membranes of bacteria much like the complement proteins do in our immune system. But what is really exciting about these antimicrobial peptide antibiotic substances is that the target bacterium is not able to mutate and defend itself against this antibiotic action.
Bacteria have survived for millions of years by developing resistance to new stressors including natural antibiotics like penicillin. What simply happens is that the bacteria, with a high rate of mutation, ends up modifying one or more of its enzymes that are used to break the link between a target protein and the antibiotic. As a result, the antibiotic does not work.
But to adapt to a peptide antibiotic that punches a hole in the cell membrane is a different story. To protect itself, the bacterium would have to change the entire composition of the cell membrane. And to change the composition of a membrane would mean changing many of the enzymes that are responsible for making the complex membrane in the first place.
